A 3D-Printed Aortic Model That Made the Difference

The case involved an 82-year-old patient with a history of EVAR performed in 2013 to treat a 60 mm abdominal aortic aneurysm. Despite multiple embolizations due to suspected type II endoleaks, the aneurysmal sac continued to enlarge until the patient was admitted in critical condition: cardiac arrest, right heart overload, and anuria. A CT scan revealed a severe type IA endoleak accompanied by an aorto-caval fistula.

IT3D Additive Manufacturing Supporting Vascular Surgery

In less than two days, our technical team produced the aortic model in PLA using the IT3D HP300 Dual Mix printer. This enabled the surgical team to modify a Cook Tx2 endograft to perform a PMEG (physician-modified endograft), a highly specialized technique in endovascular surgery.

The repair was completed by implanting a bifurcated distal Gore component, fully relining the old graft and sealing the leak. Due to severe stenosis, the left renal artery could not be cannulated, so the fenestration was occluded using a Begraft stent and an Amplatzer device.

Outcomes and Recovery Thanks to 3D Technology

The postoperative outcome was a success: no leaks were observed in the follow-up CT angiography, the patient showed progressive recovery after cardiac arrest, and serum creatinine levels stabilized at baseline values.

This case demonstrates how 3D printing in medicine and additive manufacturing in healthcare not only optimize surgical planning but also save lives when time and precision are critical factors.
